A total up to $50,000 is available for programs serving Lucas County residents; the maximum funding per project is $5,000. The grant period is July 1, 2019 to December 31, 2019. The selection committee reserves the right to adjust the amount awarded to successful applicants.

Q. Can I submit a 1023-EZ in place of a W-9 form if I am a 501c3 nonprofit organization?
A. No. We are not using the w-9 to determine your tax status. The W-9 allows your organization n=to receive payment from us if you are selected. It will also verify your Employee Identification Number and address where payment will be sent.
Q. For the Executive Summary section, should the 150 word maximum include the organizational description as well as the problem, opportunity & need?
A. The 150 word count for the Executive Summary Section should just include your executive summary. The organizational description as well as the problem, opportunity and need have separate sections which need an answer. Each heading is a separate section.
