By submitting this proposal for MHRSB Mental Wellness Community Grant funding, the applicant acknowledges and certifies to the best of their knowledge and beliefs all data and information in this proposal are true and correct, and this document has been duly authorized by the governing body of the applicant and submitted by its authorized representative. Further, the applicant certifies the project supports the MHRSB mission to inspire recovery and promote mental wellness, and if approved for MHRSB funding, the following conditions will be met:

• Project services will be implemented in a manner that acknowledges and adheres to all current COVID related guidelines as defined by the Center for Disease Control and Prevention (CDC), Ohio Department of Health (ODH) and Toledo/Lucas County Health Department in standing during the term of the project.
